Knowing the physics is necessary but not sufficient. The SPI exam is also a test of pacing, question-reading, and decision-making under a clock. Here's how to approach the exam itself, separate from the content you've studied.
Know the format going in
The SPI exam consists of 110 multiple-choice questions, administered over a two-hour window that includes a brief tutorial and a post-exam survey. That works out to roughly a minute per question on average — but not every question takes the same amount of time, so averages can be misleading if you plan around them too literally.
Note: a small number of questions on any given exam form may be unscored pretest items. You won't know which ones, so treat every question as if it counts.
Pace in passes, not question-by-question
A common and effective approach is to work through the exam in two or three passes rather than forcing yourself to finish each question before moving on:
- First pass: Answer everything you know immediately — definitional questions, straightforward recall, anything that doesn't require a calculation or careful elimination. Flag anything that needs more time.
- Second pass: Return to flagged questions. Work through moderate-difficulty items — questions that need some elimination or a quick calculation.
- Final pass: Tackle the hardest remaining questions and any calculations that need careful setup. Use whatever time is left.
The benefit of this approach is psychological as much as practical — getting through the easy questions first builds momentum and means you're never stuck on question 12 while 90 unanswered questions sit ahead of you.
Eliminate before you select
Most SPI questions present four answer choices, and most of the time at least one or two are clearly wrong if you know the underlying concept. Before trying to identify the correct answer, actively cross out the choices you know are wrong. This does two things: it narrows your odds even if you're not fully certain, and it often reveals the correct answer by process of elimination — especially on questions where two choices test opposite ends of the same concept (e.g., "increase X" vs. "decrease X").
Watch for answer choices that are factually true statements but don't actually answer the question being asked. The SPI exam sometimes includes a choice that's a correct fact about ultrasound physics, but irrelevant to the specific scenario in the stem — these exist specifically to catch students who recognize a true statement and select it without checking whether it actually answers the question.
Read the question stem twice — especially for negatives
Questions phrased as "which of the following is NOT..." or "all of the following EXCEPT..." are a known source of avoidable errors — not because the content is harder, but because under time pressure it's easy to read past the "not" and answer as if the question asked for the true statement. If you see a negative qualifier, it's worth a half-second pause to confirm what you're actually being asked to identify.
Calculation questions: set up before you solve
For Doppler equation or resolution-related calculations, write down what you're solving for and what values you're given before doing any arithmetic. A large share of errors on calculation questions come from plugging numbers into the wrong place in the formula, not from arithmetic mistakes — and that's much easier to catch if you've written the setup out rather than trying to do it in your head.
Also: the on-screen calculator provided at the testing center is basic — expect standard arithmetic functions, not a scientific or graphing calculator. Any calculation the exam expects you to do should be solvable with basic operations; if your approach to a problem requires functions a basic calculator doesn't have, you're probably overcomplicating the setup.
Don't change answers without a reason
It's a well-documented pattern across standardized tests: when students change an answer during review, the change is more often from correct to incorrect than the reverse — usually driven by second-guessing rather than new information. If you flagged a question and come back to it, only change your answer if you've identified something specific you missed the first time — a misread word, an overlooked qualifier, a calculation error. "I have a different gut feeling now" is not, on its own, a good reason to change an answer.
Manage the clock without obsessing over it
Checking the clock constantly creates its own anxiety and eats into your actual thinking time. A reasonable approach is to do a small number of time checks — for example, confirming you've completed roughly a third of the exam by the time a third of your time has passed, and again at the halfway and two-thirds marks. If you're behind at one of these checkpoints, that's the signal to move faster on remaining questions in your current pass, not to panic.
